PGY-1 PHARMACY RESIDENCY PROGRAM

Family Health Centers of Southwest Florida (FHCSWF) PGY-1 Community-Based Pharmacy Residency Program provides a comprehensive outpatient training experience at a Federally Qualified Health Center (FQHC). This program equips residents with the clinical and leadership skills needed to improve health outcomes for patients with chronic diseases such as diabetes, cardiovascular conditions, chronic kidney disease, and other conditions commonly encountered in primary care.

Throughout the residency, participants will gain hands-on experience in key areas of pharmacy practice, including:

• Pharmacy Leadership & Compliance
• Medication Therapy Management (MTM)
• HEDIS Measures and quality of care initiatives
• 340B Drug Pricing Programs and their impact on covered entities
• Centering Diabetes Care and counseling newly diagnosed patients

Residents at FHCSWF benefit from a unique, multidisciplinary learning environment, working alongside pharmacists and providers who play a vital role in population health management. Key responsibilities include:

• Conducting in-person and phone-based patient care consultations (e.g. MTM, CGM, Initial diabetic encounters, etc.)
• Leading a post-discharge medication reconciliation program for recently hospitalized patients
• Gaining direct experience in a 340B in-house pharmacy

Additionally, residents will engage in impact-driven projects, designing and implementing new pharmacist-led programs such as anticoagulation clinics and specialty pharmacy services, all with a focus on enhancing patient care and quality improvement.
