PGY-1 PHARMACY RESIDENCY PROGRAM

In-House Outpatient Pharmacy Locations: Del Prado/Cape Coral, Lehigh Acres, Estero, Downtown Fort Myers

This longitudinal rotation provides residents with hands-on experience in outpatient pharmacy operations, emphasizing clinical decision-making, patient-centered care, and pharmacy workflow management. Residents will develop proficiency in medication therapy evaluation, cost-effective prescribing and therapeutic substitutions while ensuring timely and safe medication dispensing.

Key Responsibilities:

• Assessing medication appropriateness, addressing therapy conflicts, evaluating lab values, and recommending evidence-based interventions to optimize patient outcomes.
• Managing dispensing processes, verifying prescriptions, overseeing pharmacy technicians, and ensuring compliance with 340B and privacy regulations.
• Engaging in shared decision-making, conducting medication counseling, and collaborating with providers on cost effective alternative and prior authorization resolutions.
• Leading topic discussions, implementing quality improvement initiatives, and utilizing patient-centered communication techniques to support adherence and positive health outcomes.

Core Learning Experiences

340B Contract Pharmacy Compliance

Location: Colonial office

This 5-week rotation will introduce pharmacy residents to the legal and regulatory components of FHCSWF’s Pharmacy Department. Residents will spend time with the Compliance Clinical Pharmacist and Pharmacy Compliance Administrator to learn more about contract pharmacies, 340B compliance, and other related topics such as 340B program integrity, regulatory compliance, and program vendors.

Key Responsibilities:

• Developing a basic understanding of the 340B program and compliance requirements for covered entities.
• Managing contract pharmacy claims and discuss interpretation of pharmacy policies and procedures around contract pharmacy claims.

Engage in current 340B Weekly Report updates to understand how changes in polices, laws, and manufacturer contracts affect FQHCs

Team-Based Patient Care

Clinic Location: Del Prado/Cape Coral

This 8-week rotation will provide pharmacy residents with an introduction to clinical operations of FHCSWF’s Adult Medicine and Pediatric Clinic. During this rotation, residents will have the opportunity to gain insight into the organizations structure as it relates to clinic workflow, provider care, patient populations and serve as a medication resource for both providers and patients.

Key Responsibilities:

• Observing and building rapport with providers and patients as a foundation for future pharmacy involvement.
• Developing confidence as a newly licensed pharmacist in interdisciplinary interactions and providing evidence-based recommendations to providers relating to drug selection, drug interactions, and dose adjustments.

Pharmacy Practice Management & Leadership

Location: Del Prado/Cape Coral, Colonial, Estero

This 18-week rotation will introduce pharmacy residents to the administrative and financial components of FHCSWF. This experience will teach residents how quality programs and risk adjustment can impact primary care offices while learning the processes that pharmacists in the leadership departments use to generate income for the organization. Residents will gain a more holistic understanding of the financial aspects of HEDIS measure compliance and quality-based contracts as they progress through the rotation and will be capable of generating solutions and business ideas that can benefit the healthcare entities over time.

Key Responsibilities:

• Attending, creating, and delivering presentations for different healthcare audiences including our FHCSWF Chief Pharmacy Officer, HEDIS team, Quality Assurance Quality Improvement (QAQI) team, and annual Pharmacists and Provider team meetings.
• Designing and presenting a Business Plan with a focus on evaluating financial impacts, determining sustainability of proposal and clinical patient-centered outcomes.
• Understanding the impacts of Risk Coding in primary care by discussing organizational challenges with improving risk coding and brainstorm ways to improve provider visibility to patient associated risk codes.

Ambulatory Care

Location: Lehigh Acres

This 6-week rotation provides pharmacy residents with the opportunity to collaborate with an Adult Medicine Provider to provide care in a busy, fast-paced clinic. Residents will be encounter and manage a wide variety of chronic conditions including diabetes, hypertension, hyperlipidemia, tobacco use disorder, congestive heart failure, COPD/asthma, depression/anxiety, osteoporosis, and anticoagulation.

Key Responsibilities:

• Evaluating patient charts to determine any medication related discrepancies such as medication cascades, drug interactions, dose adjustments, and/or deprescribing initiatives.
• Building confidence in communicating recommendations to providers in clinic and counseling patients on new start medications.

Identifying patients that would benefit from additional individualized care including MTM, CGM placement, counseling/education, etc
